We collect information directly from you, your family or representatives, your support coordinator, and (with your consent) health professionals and government agencies such as the NDIS.
Why we collect and use it
We use your information to deliver and coordinate your care, communicate with you, meet our obligations as a registered NDIS provider, process supports and funding, and improve our services. We do not sell your personal information.
Disclosure
We only disclose your information where necessary to provide your supports (for example to your treating professionals, the NDIA or DVA), where you have consented, or where required by law.
Storage and security
We store your information securely and take reasonable steps to protect it from misuse, loss, and unauthorised access. Access is limited to staff who need it to support you.
